Band Registration Forms

Band Registration will take place on Thursday, July 27th between 5:00pm & 8:00pm at a location TBD.  The registration process will take approximately 30 minutes to complete.  Please be prepared to pay for all band fees at the conclusion of the registration process.

Please print out the PDF files below and complete all forms to turn in at band registration.  All forms are required for participation in SFA Bulldog Band.

*We are no longer requiring students to have a documented physical on file to participate in band activities.
